Primary Care Physician and provide care services for frail and elderly
patients in long-term care settings in Georgetown and Millsboro. Its a

include: * Communicating with physicians, nursing home staff, members and
members families * Providing primary care that will focus on *****sment,
health management, education, advocacy and prevention * Entering health
history data into a web-based application
Qualifications
Only nationally certified Nurse Practitioners will be considered. National
NP certification should be in Gerontology, Adult or Family care. Experience
in a long-term care environment, or related geriatric experience, is
preferred. A background in managed care would also be helpful. Newly
graduated NPs with extensive clinical experience as RNs are also
encouraged to apply.
